There are three little objects flying around in this shot. They are most likely bugs. Niko punches a guy here and blood falls all the way to the ground. Niko is holding something else in his left hand, though. It may be a machete or other blade, but it also might be some sort of shotgun. There's a water tower to the right, but that was obvious right?
